First verify you really own yourself the domain name, then if you have to login to your account at the registrar where the domain is registered. If you don't have the information to login, you have to ask your hosting company for these information.
Once in your account, verify that your information are correct, especially your email address, and verify the domain name is no locked. If the domain is locked, you won't be able to transfert it. The registrar is certainly offering a way to unlock the domain.
Then go to the registrar where you want to transfert your domain, in their order form, you certainly have the option to transfert a domain. Select this option. Once the order made, you will certainly receive an email asking your authorization to transfert...
Hope it can help,
